Humana taps new model for nephrology SNP

Humana and DaVita are joining forces to try out a new way to serve high risk, special needs patients with a primary care-led model.

Humana is creating a Medicare special needs plan for patients with end-stage kidney disease, in partnership with VillageHealth, the integrated kidney care unit of the dialysis giant DaVita HealthCare Partners.

Humana's program, called Humana Kidney Care, will specifically target end-stage renal disease patients in greater Las Vegas and rely on DaVita Kidney Care dialysis centers in the area as well as DaVita's network of physicians and other providers -- using what DaVita calls the "total care model."

"By delivering special needs plans to Humana's chronically ill patients, we can tailor the critical aspects of their care to help meet their individual needs," said Rick Beavin, Humana's regional president of seniors services. "Coordinated care plans like VillageHealth have proven to lead to better patient outcomes and improved health-related quality of life, which is why Humana is excited to offer these plans to our Nevada Medicare Advantage members."

DaVita's HealthCare Partners in Nevada practices the "total care model," leveraging primary care providers supported by a network of nurses, specialists, care managers, diagnostic team members.

The primary care-led, interdisciplinary team coordinates all the patient's healthcare and helps them navigate the system, including any hospital services.

"We have proven that patient outcomes can be impacted through a total care approach," said Todd Ezrine, President of DaVita VillageHealth. "This special needs plan with Humana gives us the opportunity to do exactly that: help make our patients' lives better."
