TransforMED, a wholly-owned subsidiary of the American Academy of Family Physicians, has announced a new service aimed at helping small primary care practices implement its patient-centered medical home model of care.
TransforMED’s “Small Practice Package” program bundles together the necessary tools and components, and streamlines the process to enable practices with four or fewer physicians to implement the TransforMED PCMH model in two years.
It will cost a practice $1,250 per quarter to use the PCMH implementation support program. TransforMED will assess each practice’s current state using Web-based methods to determine the necessary changes to become a PCMH and then provide them with a comprehensive transformation plan and time line. Practices will also have an onsite option for $2,500 per quarter.
Each practice will be able to collaborate with others who are using the program. Those who enroll in the PCMH program will receive three individual memberships to Delta-Exchange, TransforMED’s online learning community.
As part of the training, participants will be able to attend the TransforMED Institute, which will feature education forums on how to successfully transform a practice to the PCMH model of care.
“Our experience has shown that small, independent practices with fewer decision makers are more nimble and actually have an easier time transforming to medical homes than larger practices in integrated systems,” said Terry McGeeney, MD, president and CEO of TransforMED.
“Primary care physicians in medical homes will be paid for work they currently are not paid to do,” he said. “They will be paid for managing populations of patients, coordinating their care and effecting positive health outcomes.”
