Through a new preferred business affiliate agreement, Cielo MedSolutions' customers will have access to TransforMED's practice transformation services and TransforMED will encourage medical practices implementing a patient-centered medical home (PCMH) to use Cielo Clinic's integrated patient registry, clinical decision support and population management tools.
"As Patient-Centered Medical Homes, primary care practices serve as the focal point of patient care coordination across all specialties," said Terry McGeeney, MD, president and CEO of TransforMED. "Our goal is to identify technologies that can assist practices to improve diagnostic capabilities and to enhance treatment and outcomes."
McGeeney expects that the relationship with Cielo will positive impact on both patients and physicians.
TransforMED's PCMH model combines the relationship-centered values of primary care with new technologies to improve quality of care and efficiency as well as patient and physician satisfaction, according to officials.
Cielo Clinic's registry, clinical decision support and population management software modules are designed to let providers and practices proactively manage the care needs of their patients.
The Web-based solution accommodates either paper- or electronic-based medical practices and integrates with electronic medical record systems to support "elements of the patient-centered medical home, pay-for-performance and clinical scorecarding."
