
Plano, Texas-based Heart to Heart Hospice, one of the largest hospice providers in the country, has launched a companion charitable foundation of the same name, the company announced Monday. Heart to Heart Hospice Foundation is a public charity that will offer support for patients, their families and communities through four specific programs. Those programs make up a multi-faceted approach to patients nearing the end of life, the company said.
First, the Foundation will provide a Patient and Family Need program meant to support grants that will help patients and their families struggling with quality of life issues due to financial hardship.
Second, an Education and Scholarship Program will endow scholarships in areas like hospice, gerontology and palliative care. It will also offer education and training for caregivers and the community in end of life issues.
A Veterans Care program will give financial support for those servicemen and servicewomen facing "life-limiting" illnesses. It will also offer support for things such as counseling, legal services, transportation, legacy programs and education grants for veterans organizations, among other services.

Finally, the Alzheimer's Support program will provide financial support for patients with late-stage Alzheimer's disease and dementia, coupled with grants and continuing education health care professionals, as well as caregiver support programs.
After providing more than $72,000 grants to patients and their families in 2015, the Foundation is slated to more than triple that amount in 2016 in scholarships to Alzheimer's, veterans and cancer support, as well as grants for patient and family needs and community education in end of life issues, the company said.
